Encephalitis

What is it?

Inflammation of the brain leading to brain dysfunction, brain damage and can be fatal.

  • It can occur anytime (not always after an infection), it may take time to develop because it maybe slowly reacting to a weak immune condition.

Main countries affected are (though it can occur anywhere):

  • Bangladesh (Niphah encephalitis)
  • India

Australasia

  • Kunjin Virus and Murray Valley Encephalitis

Ibuprofen and other NSAIDs can be used under medical supervision, however Ibuprofen and other NSAIDs is not recommended in asthma or asthma type symptoms. Check with your Medical doctor/Pharmacist when one can have any type of fever reducing medication to make sure it suitable for you. Do not give Ibuprofen and other NSAIDs in chicken pox (without medical advice) and dengue.

Cause

  • Autoimmune diseases when immune system our body defence attacks its own parts of body in this case the brain.

  • Reacting to a medication that makes the immune system weak
  • Nipah Encephalitis- Consumption of raw date sap contaminated by fruit bats and from pigs (mainly in Bangladesh, India)
  • Glutaric aciduria type I
  • Liver disease
  • Liver condition
  • MOG antibody disease
  • Precocious puberty (early stage puberty) – appearance of secondary sexual characteristics before the age of 8 years in girls and 9 years in boys. Secondary sexual characteristics include breast and/or pubic hair development in girls, and testicular enlargement and/or pubic hair development in boys. Other signs that may accompany pubertal onset include vaginal discharge or bleeding (oestrogen effects), acne, voice deepening (androgen effects), growth acceleration, and advanced skeletal maturation (both oestrogen and androgen effects).
    • Central precocious puberty is gonadotropin-dependent and occurs as a result of early activation of the hypothalamic-pituitary-gonadal axis i.e. for girls symptoms include: breast development and vaginal discharge or bleeding and for boys breast development and testicular enlargement.
    • Peripheral precocious puberty (precocious pseudopuberty) – gonadotropin-independent and occurs because of excess production of sex hormones from the gonads, the adrenal glands, or ectopic or exogenous sources i.e. symptoms include: Girls with contrasexual peripheral precocious puberty exhibit signs of virilization (pubic or axillary hair development, acne, clitoromegaly) due to androgen hormone excess (as in this patient), whereas boys with contrasexual peripheral precocious puberty show signs of feminisation (gynecomastia) due to oestrogen hormone excess.
      • Causes include:
      • Ovarian cysts or ovarian tumours (both of which typically secrete oestrogen)
        • Rare forms of ovarian tumours, such as gonadoblastomas, secrete androgen (females) androgen-secreting adrenal tumours and/or congenital adrenal hyperplasia (male and female).
        • Beware of symptoms of Cushing syndrome
  • Rasmussen encephalitis
  • Subacute sclerosing panencephalitis

Syndromes

Symptoms

Initial symptoms:

  • High temperature greater than 38°C (100°F) or over and/or chills and sweats longer than 72 hours?
  • Headache which is on and off or a constant headache longer than 24 hours?
  • Aching, painful muscles, muscle cramps/spasms/stiffness?
  • Sore throat for longer than seven days or more?
  • Swollen lymph glands and/or reoccurring mouth ulcers?
  • Abdominal pain and/or are constant violently vomiting or vomiting longer than two days (one day if a child)?
  • Diarrhoea?

Further symptoms (can occur suddenly or later depending when infection/toxin reaches the brain):

  • Confusion and/or drowsiness?
  • Loss of concentration and/or memory loss?
  • Dislike to lights (photophobia)?
  • Can’t speak and/or slurred speech?
  • A sweet odour to the breath?
  • Stiff neck?
  • Irritated and/or change of behaviour and/or  aggressive?
  • Can’t control body movement and/or shaking or jerking of the limbs?
  • Flapping of the arms when held out straight?
  • Slowed reaction time?
