Monilethrix

What is it?

Monilethrix is a rare genetic hair disorder causing brittle, sparse hair with a distinctive “beaded” appearance under a microscope due to periodic constrictions and normal-thickness nodes on the hair shaft, leading to easy breakage, often starting in infancy on the scalp’s nape but sometimes affecting eyebrows, eyelashes, and body hair.

It often starts at the back of the head and then spreads.

Causes

Family history:

Primarily inherited as an autosomal dominant trait, meaning one copy of a mutated gene from a parent is enough.
Gene Mutations: Caused by mutations in keratin genes, particularly KRT81, KRT83, and KRT86.
Recessive Form: Autosomal recessive monilethrix is often linked to mutations in the DSG4 gene (desmoglein 4)

Symptoms

Beaded Hair: The most defining feature, where hair strands look like a necklace?
Hair Fragility: Hair breaks easily, causing patchy hair loss (hypotrichosis) or “stubble”?
Early Onset: Usually develops in the first few months of life as lanugo hair is replaced?
Often starts at the back of the head (occiput) and nape, but can spread?
May include red follicular bumps (hyperkeratosis), loss of eyebrows/eyelashes and sometimes nail dystrophy (koilonychia) or other ectodermal issues?
