Prostate Enlargement

What is it?

A non-cancerous growth of the prostate gland, it then presses against the bladder and urethra (the tube where urine flows) and affects the flow of urine.

  • Symptoms can vary in range and does not increase the chance of prostate cancer or is serious to men’s health.

Diagnosis Tests

There are variety tests to confirm this condition from physical examinations to the  detecting levels of prostate-specific antigens in the blood (please note this test is not specific to detecting cancer of the prostate merely an indicator).

  • Ultrasonography (abdominal, renal, transrectal) and intravenous urography may also be included to determine bladder and prostate size and as well as to also check for hydronephrosis.

  • One test that should be considered is if a patient has predominant significant nocturia (urinating at night) and is awakened two or more times per night, a frequency volume chart (also called a voiding diary) covering 2-3 days is recommended
    • Nocturnal polyuria (ration of urine at night compared to day) is diagnosed when more than 33% of the 24-hour urine output occurs at night.
    • Patients with bothersome symptoms should aim for a urine output of 1L per 24 hours.

Symptoms

(Symptoms come and go, over a period of time)

Men only:

  • Anaemic (i.e. lack of oxygen in blood, due to lack of iron, folic acid or heavy blood loss)?
  • Blood in urine?
  • Difficult to start urinating and stop urinating?
  • General discomfort (muscle weakness), uneasiness or ill feeling (malaise) and/or fatigue (tiredness)?
  • High temperature greater than 38°C (100°F) or over and/or chills and sweats longer than 72hours?
  • Feeling bladder has not fully emptied after urinating making infections (cloudy-smelly urine, fever, nausea and vomiting, pain in abdomen) more likely?
  • Aching, painful muscles, muscle cramps/spasms/stiffness?
  • Joint stiffness and pain?
  • Increase thirst, heavy sweating and need to urinate frequently?
  • Abdominal pain and/or are constant violently vomiting or vomiting longer than two days (one day if a child)?
  • Pain in pelvis area, lower back?
  • Pain when urinating?
  • Strain to urinate due to weak flow?
  • Erectile dysfunction?

  • Dehydration symptoms
  • Unable to pass urine
  • Severe pain and swelling in abdominal region?
  • Symptoms of  Acute (short term) Prostatitis developed SUDDENLY:
    • Severe abdominal pain?
    • Frequent need to urinate?
    • High fever of  38°C (100°F) , feeling hot sweaty and clammy?
    • Nausea and vomiting?
    • Swollen scrotum?
    • Pain when urinating?
    • Difficult to start or stop urination?

Methods to reduce lower urinary tract symptoms:

  • Avoid alcohol and caffeine
  • Avoid drinking fluids at bedtime, and drink smaller amounts throughout the day
  • Avoid taking decongestant and antihistamine medications (especially if one believes they have cold and flu symptoms)
  • Get regular exercise
  • Make a habit of going to the bathroom when the urge is present
  • Practice double voiding (empty the bladder, wait a moment, then try again)
  • Practice stress management and relaxation techniques

  • Regularly practise pelvic floor exercises to strength the muscles that control urine flow;

  • Stop smoking to reduce the incidence of coughing, which can strain the pelvic floor muscle;
  • Avoid heavy lifting. This can put pressure on the pelvic floor muscles, increasing the risk of leaks.

  • Reduce caffeine intake to limit irritation to the bladder;

  • Decrease alcohol consumption as alcohol acts as a diuretic;
