Osteomyelitis

What is it?

Infection of the bone.

  • It is the inflammation of the fatty tissues within the bone and can occur on any bone i.e. osteomyelitis pubis – Inflammation of the fibrocartilaginous pubic symphysis joint (in middle of hips)

Diagnosis Tests

  • MRI  (show presence of fluid and extensive soft-tissue reaction)
  • Radiologically guided needle aspiration and culture

Cause

In neonates (babies)

  • Bacteria: Streptococci type B, E.coli

Children

  • Bacteria: Stapphylococcus auerus, Streptococci type A, Haemophillia influenza type B, Mycobacterium ulcerans bacteria causing buruli ulcer

Adults

  • Bacteria:
    • Stapphylococcus auerus
    • Other isolated cases include: Streptococcus viridans, Staphylococcus epidermidis, and Salmonella species.
    • Pseudomonas aeruginosa (mainly in illicit drug users)
    • Brucellosis
    • Tularaemia
  • Botryomycosis
  • Fungus: Coccidioidomycosis, Blastomycosis, Cryptococcosis, Candidiasis, Sporotrichosis

  • Urologic or gynaecological manipulation i.e. hysterectomy (osteomyelitis pubis)
  • Surgery
  • Kidney dialysis

Illicit drugs

Symptoms

  • High temperature greater than 38°C (100°F) or over and/or chills and sweats longer than 72 hours?
  • General discomfort (muscle weakness), uneasiness or ill feeling (malaise) and/or fatigue (tiredness)?
  • Fast heart beat?
  • Abdominal pain and/or are constant violently vomiting or vomiting longer than two days / one day (if child)?
  • Pain in affected bone area (worsen with exercise)?
  • Lack of movement in area or no movement of area?
  • Bone(s) appear deformed or unusual?
  • Swelling (can be fluid filled) in affected area?
  • Feel hot in affected area?
