What is it?

  • Warfarin (Coumadin , Jantoven)
  • Acenocoumarol (Sinthrome)

Complications /Information to beware of/General tips:

  • Warfarin has a delayed onset of action (maximum effect occurs up to 48 hours after administration) and prolonged effect while Acenocoumarol has short acting affect (maximum affects occur after 1 to 3 hours following oral administration).
  • Target INR ranges depend on indication. INRs less than 1.5 increase risk of thrombosis and over 3.5 increase risk of intracranial haemorrhage.
  • Patient should know and understand their warfarin dose.
